Your visit

Meet with Robert Carroll, MSN, FNP‑BC, a board-certified Family Nurse Practitioner and the sole clinician at Carroll Health & Wellness, for a scheduled video consultation. Visits are for adults 18 and older who are physically in New Jersey during the appointment.

Carroll Health & Wellness provides personal care for common, non-emergency concerns that can complement your usual primary care. Each visit focuses on the concern being addressed. Ongoing management of a condition starts only when you and Robert explicitly agree on a plan. Robert remains responsible for follow-up related to the care he provides, including his orders, results and prescriptions.

Visits are scheduled for 15 minutes. Robert may allow additional time when needed to complete the discussion and establish clear next steps, without an added fee.

If you prefer a telehealth visit with a physician, Carroll Health & Wellness can assist you with arranging an appointment with an outside physician through Zocdoc.

Appointment requests are reviewed for eligibility, clinical appropriateness, practice scope and scheduling availability. Submitting a request does not guarantee that an appointment will be accepted or confirmed.

What your $123 consultation includes

There is no membership or subscription fee. Each separately agreed consultation is $123.

There are no practice add-on fees for these included services or Robert’s permitted time extension. Medicines, actual testing, imaging and care from outside providers cost extra.

A new concern or an agreed ongoing-care plan may require a separate consultation. Each separately agreed consultation, including ongoing high-blood-pressure or diabetes follow-up, costs $123. The appointment and fee are explained and agreed before booking. Included results follow-up is not automatically converted into another paid visit.

Recommendations depend on the clinical assessment. A visit does not guarantee a prescription, test order or medical note.

Routine paperwork directly related to your consultation is included when supported by your assessment and available records. If you need a specific form completed, let Robert know before your visit so he can confirm whether it can be addressed through this practice.

Payment and insurance

Carroll Health & Wellness is a self-pay practice. We do not accept or bill insurance for consultations, including Medicare or Medicaid. After Robert reviews your request and confirms your appointment, pay online by credit card using the practice’s secure payment link before the consultation. Flexible spending account (FSA) payments are not accepted. A payment receipt is available.

Carroll Health & Wellness does not offer consultations to patients enrolled in Medicare, Medicare Advantage, Medicaid or NJ FamilyCare, including self-pay visits. Please use a provider who accepts your coverage.

You may use insurance separately for laboratory tests and prescription medications through an outside laboratory or pharmacy that accepts your plan, subject to coverage and plan requirements. An imaging center may also bill insurance separately. Confirm coverage and charges with each outside provider. Review the financial terms before payment and ask about any charge you do not understand.

Cancellations, missed visits and refunds

Please let the practice know as soon as you need to cancel or reschedule. There is no minimum cancellation-notice period and no additional cancellation, late-cancellation or missed-visit fee.

If no consultation is completed and you have already paid, you receive a full refund. This includes a cancellation, missed appointment, practice cancellation, failed video connection, being outside New Jersey, or arriving too late for a safe assessment.

The $123 fee applies when Robert completes an assessment and provides a care plan, including a recommendation for in-person care or a decision that a prescription is not appropriate. If a safe assessment cannot be completed, the full-refund rule applies.

Refunds are returned to your original payment method. Your bank or card processor determines when the money appears in your account.

Use your patient portal for an appointment change. If you cannot access it, email Robert.FNP@​CarrollHealthWellness.org for administrative help without including medical details. A replacement appointment time must be agreed and confirmed.

Choose an available appointment time and submit your request. Robert reviews it before confirming the appointment. Check your confirmation for the final date and time.


Connect your portal and complete your forms

Use the practice’s Charm invitation to connect your patient portal account. Complete the requested forms and review the consent, privacy information and financial terms provided for your visit. These are separate steps.

The current intake questionnaire is called “A few questions before your visit.” You may save a draft and return to it. When finished, choose Submit and check for Submitted. Saved or Draft means it has not been submitted.

Pay before your visit

After your appointment is confirmed, pay the $123 consultation fee by credit card using the practice’s secure payment link. Payment must be completed before the consultation can begin. If you are unsure whether payment succeeded, use the patient portal for help. Never send card numbers, security codes or passwords in messages.

Get ready for video

Open the visit link in your appointment instructions or patient portal. Use a phone, tablet or computer with a camera, microphone and reliable internet. Join from a private location in New Jersey. Have your medicines and relevant records available. Robert will confirm your physical location and private contact details.

Tests and results

This result work is included in the consultation fee. The practice remains responsible for reviewing and communicating its results when another clinician will provide ongoing care.

Prescriptions and costs

Your prescription plan may cover a medicine, depending on your coverage and pharmacy. Coverage is not guaranteed. Confirm your out-of-pocket cost before filling the prescription.

If you are uninsured or paying out of pocket, Robert can provide a GoodRx coupon when one is available. GoodRx is not insurance and cannot be combined with insurance for the same purchase. Ask your pharmacy to compare the available payment options and confirm the final price.

For selected erectile-dysfunction prescriptions, the practice can help arrange discounted pricing through Drug Mart in South Plainfield, which offers mail order. You may choose your pharmacy. Confirm the medicine’s availability, price and delivery details before filling it.

Help, connection problems and response expectations

Use the patient portal for medical questions, photographs, records, appointment changes and patient-specific payment questions. Email Robert.FNP@​CarrollHealthWellness.org for general scheduling, registration, portal-access or connection help, without medical details. Routine correspondence is in writing.

Nonurgent portal messages are generally reviewed within 1–2 business days. Response times may be longer on weekends and holidays. The patient portal should not be used for urgent or emergency concerns. Resolving your concern may require more information or a visit. Email is not an emergency service; for a medical emergency, call 911.

Routine scheduling, registration and billing questions receive a substantive reply by 5 p.m. on the second business day after receipt. Business days are Monday through Friday, excluding U.S. federal holidays; the next business day is day one. A reply may answer your question or explain the next step. It does not itself confirm an appointment or complete a refund.

If you do not yet have portal access, ask for registration or secure-access help by email. You do not need to describe your symptoms in ordinary email to get help connecting.

If your video connection drops, try the visit link again. Message the practice through the portal if you cannot reconnect. If the portal is unavailable to you, use the administrative email for connection help. The practice must confirm any new connection time or rescheduled appointment.

If you have difficulty reading a page or completing a step, tell the practice which page or step is causing difficulty and what would help. Keep medical details out of ordinary email.

Cancellations and refunds

Self-pay and privacy

Carroll Health & Wellness does not submit consultation claims to health plans, so the practice does not generate an explanation of benefits (EOB) through an insurance claim for your visit.

Outside laboratories, imaging centers and pharmacies bill separately. If they bill your insurance, or you submit your receipt for reimbursement, your plan may receive information about the care and issue an EOB or other notice. Discuss self-pay and communication preferences with each service before it processes your care.

Information may still be shared for treatment, necessary practice services, with your authorization where required, and as permitted or required by law, including public-health reporting. Tell the practice through your portal if you need communications handled in a particular way.

Privacy & records

Good Faith Estimates

A Good Faith Estimate is a written breakdown of the services and charges expected for your care. It is not an extra fee or a bill. Eligible patients who are uninsured or paying without insurance can ask for one before deciding whether to book.

Save your estimate and compare it with your bill. If a provider or facility bills you at least $400 more than that same provider or facility estimated, you may qualify for the federal billing-dispute process.

To ask, message the practice through your patient portal or email Robert.FNP@​CarrollHealthWellness.org and request a Good Faith Estimate. You do not need to book or pay first. Keep medical details and patient-specific billing information in the portal.

Your consultation fee is $123. Your individual estimate explains the expected services and charges for your planned care; this webpage and your payment receipt do not replace it. Outside laboratories, imaging centers, pharmacies and other practices charge separately. Ask each for its expected costs and any applicable estimate.
