Carroll Health & Wellness LLC maintains your patient records. Robert Carroll, MSN, FNP‑BC, is the practice’s privacy official. You may request access to your records, ask for corrections, ask us to send records to another healthcare provider, or raise a privacy concern.
For privacy questions and help with records requests, call 727‑272‑7178 or leave a voicemail, or email Robert.FNP@CarrollHealthWellness.org. Use your patient portal for sensitive information and documents. Routine clinical questions are handled through the portal.

If you have difficulty reading this website or completing a step, ask for assistance through the portal or administrative email. Tell the practice which page or step is causing difficulty and what would help, without sending medical details by ordinary email. If you do not yet have portal access, the practice can help you obtain secure access.
